POSITION: Womens Health Assistant Case Manager
STATUS: Non-Exempt
IMMEDIATE SUPERVISOR: Womens Health Manager/Care Management Coordinator
Receives all pap smear results for health center patients. Maintains tracking system to ensure that results are received for all pap tests obtained in the clinic. Researches past medical history for history of abnormal pap.
Schedules patients for colposcopic exam or repeat pap smear according to Joseph Smith protocol for the management of abnormal pap smear results. Contacts patients to explain the purpose of the test and what to expect at the time of the visit. Reminds patients of upcoming appointments.
Prepares for colposcopy clinic sessions by gathering pathology and cytology results as well as any pertinent records from other facilities. Prepares the medical record for the session.
Escorts patients to examination room. Records the appropriate history of the presenting gynecological problem and the appropriate past medical and social history in the electronic medical record. Measures and records vital signs. Incorporates information from pathology reports and/or notes from other providers into the HPI for current visit.
Assists provider during examinations. Prepares specimens for transport to outside laboratories and completes all necessary forms. Washes, wraps and prepares all instruments for sterilization. Inspects instruments for defects and informs appropriate staff when repair or maintenance is required. Responsible for maintaining inventory and supplies.
Provides translation and emotional support to patients as needed during procedures and treatments.
Updates and maintains data base for management of abnormal pap smears. Regularly reviews case management notes with provider supervisor. Identifies patients who are overdue for evaluation or services. Makes regular contact by telephone and by regular and certified mail as indicated to ensure appropriate medical visits are scheduled and attended. Schedules outside appointments, as requested by provider and informs patient of same including information needed to prepare for exams.
Assists the case manager in the breast and cervical cancer programs of the Womens Health Network Allston/Waltham sites to assure that services are generated in a time and cost effective manner.
Documents all case management activities in client record according to policy. Assists the case manager, clinician, program coordinator, and client navigator to enroll, assess, plan, and discharge clients from case management per the WHN Case Management Policy.
Organizes and maintains language appropriate patient education and referral information. Provides this information to patients at the time of procedures and treatments, or in advance of appointments as needed.
Responsible for training and orienting of new cervical cancer case management assistant staff.
Works collaboratively with other Health Center staff in meeting JCAHO accreditation and other regulatory and/or funding requirements. Performs other duties as directed. Conforms at all times to HIPPA and health center patient confidentiality guidelines.
Bachelors degree preferred.
Mature, responsible individual with strong organizational skills.
Experience in use of computer systems preferred.
Good verbal and written communication skills in English required.
Good verbal and written communication skills in Spanish preferred.
Experience in diverse cultures preferred.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
